For Entrance Exam

• Original or Certifies true copy of scholastic record (Duly Authorized by the Department of Foreign Affairs)
• Accomplished Personal History Statement (PHS) with photo (white background) and right thumb print, duly signed by the applicant
• Affidavit of adequate support to cover expenses incidental to studies
• Authenticated Police Clearance Certificate Issued by the National Police Authorities in the Student’s Country of Origin
• Quarantine Medical Clearance Certificate Issued by the National Quarantine office
• Photocopy of passport (with page stamped with latest arrival and visa)
• P200 entrance exam fee

For Enrollment

• Certificate of Good Moral Character (Original Copy)
• Transcript of records (Original copy)
• Honorable Dismissal (Original copy)
• NSO Birth Certificate (2 photocopies)
• Two pcs 2X2" ID photos white background
• Four pcs 1X1" ID photos white background
